Our long-term goal is to create and sustain a public dialogue that will measurably improve the “systems” that finance and provide health care in New Hampshire in order to accomplish two fundamental objectives:

Governor Lynch, NH Health Care Providers Announce Innovative Health Care Pilot Program Aimed at Improving Quality, Lowering Costs

New Accountable Care Organization Pilot Program Part of Governor's Continuing Efforts to Help Keep Health Care Affordable, Accessible for Families and Businesses

CONCORD - Governor John Lynch today joined members of the Citizens Health Initiative, health care providers and insurance industry officials in announcing a new pilot project aimed at encouraging health care providers to collaborate and focus more on prevention and disease management, which will result in healthier patients and reduced growth in health care costs.

Health care providers and New Hampshire's major insurance companies from five locations across New Hampshire have agreed to participate in this new innovative pilot program. This pilot will move New Hampshire away from the fee-for-service model and instead give health care providers incentives to spend more time with their patients, to work with their patients to prevent new illnesses and better manage existing illnesses, and to collaborate better with other health care providers.

Pillar Projects

Health Promotion Disease Prevention

Over the decade long initiative, the work of the Health Promotion and Disease Prevention Project will focus on decreasing the leading causes of illness and death among New Hampshire citizens. We will do this through collaboration, evidence-based information, and sound policy recommendations to strengthen the public health system and will work towards increased health and well-being of all New Hampshire citizens.

Health Payment Reform

In 2009, some of the NH Citizens Health Initiative’s most important work will be centered on the design and implementation of a payment system that reflects our public and personal health goals.  We believe that a thoughtful design, constructed by a multi-stakeholder team of NH healthcare leaders and done so in consideration of the best performing systems as well local system nuances, is critical in gaining consensus and endorsement by all affected constituents.

Medical Home

The NH Citizens Health Initiative Multi-Stakeholder Medical Home Pilot represents a collaboration among the Initiative  medical home workgroup, the Center for Medical Home Improvement and the four private NH Health Plans: Harvard Pilgrim Health Care, CIGNA, Anthem, and MVP Healthcare as well as NH Medicaid. The goal of the pilot is to value, prescribe and reward medical care that is tightly coordinated and of superior quality and efficiency. The Initiative will convene and provide project management for the 2-year medical home pilot, with payment scheduled to begin on 06/01/2009. Facilitation of the project will have two foci, pilot operations and collaboration among participants selected for inclusion in the pilot.
